Services offered by the Children’s Health Alliance:

Led by a board of pediatric providers in the community with a shared vision of creating a collaborative environment of improvement among member practices to improve care for all children.
Children’s Health Alliance partnered with a leading Population Health Management system to develop a pediatric focused Care Management, Analytics, & Reporting Tool (CMART) to facilitate access and ownership of clinical data for our pediatric members, and to provide a 360-degree view of patient information at the point-of-care to enable proactive management of pediatric care protocols and improve performance to key pediatric care goals. This web-based system aggregates clinical and claims data serving as a pediatric health information exchange enabling performance reporting with health plans while also offering clinical value to providers via care gap alerts, performance reporting, registry and shared care plan functions. This actionable information in the hands of providers empowers pediatric care teams to more effectively and proactively manage care for their patients, especially children/youth with complex medical conditions and/or family and social factors affecting their health management.
The Children’s Health Alliance performs measurement and reporting of Key Pediatric Quality Measures demonstrating quality pediatric care and utilization results of our pediatrician members and their clinics. Within our clinically integrated network, we report performance transparently to encourage collaboration and adoption of improvements. Key to our measurement strategy is access to real-time, actionable and trusted clinical data that is aggregated from all members Electronic Medical Record systems as well as from payer claims and enrollment systems as well as hospital admission registries. Reporting of trusted clinical performance data enables action at the point of care, and enables a broader population view of care for children in our community. Our pediatric Population Health analytics and HIE tools also enable us to centrally report results for contract quality programs and state measurement requirements to offload this work at the individual clinic level.
Consultation on medical home optimization, collaboration with the Oregon Patient Centered Primary Care Home program, and assistance with quality improvement projects to enhance the medical home. Coordination with pediatric practices to complete patient experience of care surveys and interpret results. Opportunities to participate in shared learnings to enhance pediatric care management and integrated behavioral health in a pediatric setting.
Partner with health plans and coordinated care organizations to provide collective value in value-based contracting for the pediatric population.
Advocacy on behalf of pediatric practices, children and families, including representing the pediatric voice in state advocacy for meaningful pediatric measures and benchmarks, and pediatric representation in key industry forums.
